Autorización para uso de fotografías y testimonios
Mediante la presente yo autorizo a Common Wealth Charlotte para usar mi imagen y testimonios (historia) en fotografías, videos o cualquier otro medio digital (foto) en todas y cualquiera de sus publicaciones, incluidas publicaciones en internet, sin ningún pago ni remuneración.
Yo entiendo y accedo que las fotos y testimonios serán propiedad de Common Wealth Charlotte y no pueden ser regresados.
Mediante la presente yo autorizo a Common Wealth Charlotte a editar, modificar, copiar, exponer, publicar o distribuir estas fotos y testimonios para cualquier propósito licito y legitimo. Además, cedo cualquier derecho a inspeccionar o aprobar el producto final en donde mi retrato o testimonio sea utilizado. Adicionalmente, cedo mi derecho a regalías u otro tipo de compensación que surja o esté relacionada al uso de mi imagen.
Mediante la presente exonero y relevo de responsabilidad por siempre a Common Wealth Charlotte de cualquier reclamo, demanda y acción en la cual yo, mis herederos, representantes, encargado de herencia, administradores, o cualquier otra persona actuando en representación mía o en representación de mi herencia tenga o pueda tener razones para esta autorización.

Photo and Testimonial Release
I hereby grant Common Wealth Charlotte permission to use the attached likeness and testimonial (story) in a photograph, video, or other digital media (“photo”) in any and all of its publications, including web-based publications, without payment or other consideration.
I understand and agree that all photos and testimonials will become the property of the Common Wealth Charlotte and will not be returned.
I hereby irrevocably authorize Common Wealth Charlotte to edit, alter, copy, exhibit, publish, or distribute these photos and testimonials for any lawful purpose. In addition, I waive any right to inspect or approve the finished product wherein my likeness or testimonial appears. Additionally, I waive any right to royalties or other compensation arising or related to the use of the photo.
I hereby hold harmless, release, and forever discharge Common Wealth Charlotte from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have by reason of this authorization.
